Signatures must be original, FDA approved digital signatures, or must be completed using the form field provided. In accordance with the GDP, pictures of signatures are not acceptable.
Upon completion of the form(s), return to the contact in the original email. Any indicated log numbers will be completed by Teleflex, please do not enter them. If additional documentation was requested, feel free to attach those along with the request form(s).
